Photo Release Form

• Any photos taken during a training session will be sent to each attendee and or the Salon. It may be used on Social Media by both parties. 
• All photos are subject to our Safe Photo Policy.
• No photos or video are to be taken by other staff, attendees or salon owners. 
• Only paid attendees and models will be allowed in the training session. 

Terms and Conditions: 

• By submitting this form I am signing up for the selected course/s, with training fees payable as per the invoice.
• No flights will be booked before the deposit is paid. Change in costs, due to delay in payment will be added to the invoice.
• The balance of the account to be settled to receive manuals, course materials and the login in details to your online course.
• You will receive the login to your online course by email and your manual is printed from there. 
• Once the manual, course materials & log in details has been sent, no refunds will be made.
• If illness or unforeseen circumstances causes a need to postpone training, the course fees will be transferred to an alternative training date. Where applicable, all flight and accommodation changes needed will be covered by the salon.
• We take pride in education; therefore all course pre-requisites must be met prior to any post-graduate training course being attended. Please don’t be offended if we verify your qualifications. Please email a copy of your Diploma in Beauty Therapy to salonsetupnz@gmail.com. Please use your full name as reference.
• Salon Setup holds no responsibility for any issues/damages/injury that may occur as a result of the trainee conducting treatments in the salon.
• Salon Setup reserves the right to alter, cancel or postpone courses as we see fit.
• No video recording is allowed during training. Before and After Pictures will be taken by the trainer and sent to the salon or therapist.
• No part of the training may re-produced, duplicated or sold. 

I ACKNOWLEDGE THAT I HAVE READ, UNDERSTAND & AGREE TO THE ABOVE TERMS & CONDITIONS, HAVING SIGNED IT FREELY AND VOLUNTARILY. 

CONSENT FOR TRAINING AND WAIVER OF LIABILITY for ALL SALON SETUP TRAINING COURSES.

1. I understand, acknowledge, and consent to being trained and/or treated in the practice of the above-indicated course/s by the appointed Salon Setup Educator and Professional Trainer (“Trainer”) and Students (“Student”).

2. Being trained in the above indicated methods, I understand that I will be working with certain surgical instruments and/or chemical solutions 
and understand the risks and hazards related thereto.

3. I understand that I will be undergoing a treatment by a Student who is not fully skilled in the practice of the abovementioned method/s but is under the guidance of a Trainer.

4. I understand the risks, hazards, and complications of using surgical blades or needles, having surgical blades or needles used upon me which are very sharp and can cause cuts, lacerations and other injury, including blood loss and infection.

5. I understand the risks, hazards, and complications of using or having used upon me certain chemical skin preparation solutions.

6. I hereby release, indemnify, and hold harmless Salon Setup, a New Zealand Company, its respective directors, officers, employees, agents, trainers, contractors and successors and assigns from and against any and all liability for any loss, injury, expense, demand, or cause of action that I may suffer with respect to personal injury, death, or damage to myself that may arise as a result of my training in any of the abovementioned methods, during training class or afterwards while offering these methods to my clients.

7. I consent to the presence of observers to my being trained and/or treated in the practice of any of the above-indicated methods as approved by the Trainer.

8. I consent to the photographing or videotaping of my being trained and/or treated in the practice of any of the abovementioned methods, so long as only appropriate portions of my body are depicted and my identity is not revealed. I agree not to produce any recordings.

9. All parties agree and consent that all disputes, claims and controversies relating to this Agreement and the relationship of the parties hereof shall be brought in the exclusive forum of the courts, and the parties agree that the prevailing party shall be entitled to recovery of reasonable attorney’s fees and costs.

I ACKNOWLEDGE THAT I HAVE READ, UNDERSTAND & AGREE TO THE ABOVE CONSENT FOR TRAINING AND WAIVER OF LIABILITY, HAVING SIGNED IT FREELY AND VOLUNTARILY.
